Our family doctor does not warrant or guarantee network participation or insurance benefits. Any costs incurred by the patient for services rendered are the responsibility of the patient. Payment according to insurance policy rules (i.e.: co-payment) is required at the time of service.

  • Every insured patient needs to confirm that our family doctor Physicians are part of his/her insurance plan or network.
  • Patients having insurance that we are not contracted with may incur additional out-of-pocket expense.
  • Please call the customer service number on your insurance card to verify our participation in your insurance plan.


Not having insurance does not mean not having a doctor! Cash, check and credit card payments are accepted. Call and ask about self-pay options. Payment is required at time of service.
